Important information for All Applicants:
- A valid Social Security Number is required for the primary applicant responsible for the Account. Positive ID verification is required of all Applicants to complete the application.
- Applicants and Person(s) Authorized on the application must be 18 years of age or older to apply for service and conduct business on behalf of the Primary Applicant.
- All applications are reviewed and processed by a Customer Service Representative. Incomplete or invalid data may result in delays of processing your request for new service.
- Additional documentation may be requested in order for the application to be processed.
- Submitting this application does not guarantee service.
- Services will be connected Mon-Fri from 9 a.m.- 6 p.m. (excluding weekends and holidays).
- Allow two (2) business days to process your application. Dates for requested connection are based on availability and are subject for connection on the next available business day.
- A service fee of $24.56 will apply on the opening statement of the account. Same day requests are subject to an additional fee of $49.13.
